Enrollment Made Simple
Ready to join the DLA family? Follow these simple steps to complete the online enrollment process. Our team is here to guide you through every step — from forms to final approval.
Dayton Leadership Academies will require parents/guardians to complete online Student Demographic, EMA (emergency medical authorization), and income verification forms through the OneView system. These online forms replace the paper forms that were distributed in prior school years. The information contained in the online forms helps ensure that we have the most up-to-date information pertaining to your child. In addition, should any information pertaining to your child change throughout the school year, we ask that you update the information using the OneView system, send us an email or text, or bring any changes into the office.

Step 1 - online forms
Complete the online enrollment forms by clicking on the link at the bottom of this page. Completion of the enrollment forms will take approximately 20-30 minutes. If you need assistance or have questions please contact Shelicia Brown, Family and Community Engagement Coordinator at 937-361-3175. Your call will be returned in a timely manner. -
Step 2- Submit Required Documents to Finalize Enrollment
At the end of the enrollment application, you will have the option to upload required documents. These documents can also be sent via email to sbrown@dlaeagles.org as attachments with your child’s name listed in the subject line. Pictures of the required documents will also be accepted as long as the information is clear.
The following documents are required for enrollment and missing or incomplete documents will result in a delay of enrollment:
Copy of birth certificate
(1) Proof of residency. Options include:
Lease agreement
Mortgage statement
Utility bill (no more than 60 days old)
Tax document
Document from job and family services
Voter registration
Paycheck stub
Bank statement
Copy of parent/guardian ID or driver’s license
Shot records
Court-stamped custody documents (if applicable)
IEP / MFE / 504 Plan (if applicable)
