Dayton Leadership Academies will require parents/ guardians to complete online Student Demographic and EMA (emergency medical authorization) forms and income verification forms utilizing the OneView system. These online forms replace the paper forms that were distributed in prior school years. The information contained in the online forms helps ensure that we have the most up to date information pertaining to your child. In addition, should any information pertaining to your child change throughout the school year, we ask that you update the information using the OneView system, send us an email or text or bring any changes into the office. 

Please note that each school year you are required to update your student’s Proof of Residency.  

Current students are not re-enrolled and New students are not accepted until the proof of residency is uploaded into OneView.  

This is a required field and your forms will not be processed until the proof of residency is received.  This also means your student’s seat will not be saved until this process is completed.
